Information Regarding Consent for Health Related Services |
In accordance with law, the District must provide parents with written notice of each school-based health-related service offered at the campus their child attends. These routine services promote student safety, wellness, and readiness to learn. The services may be provided by qualified school staff, including nurses and athletic trainers. This consent does not take the place of an individualized health plan, 504 plan, or other legally required document. A parent has the right to withhold consent for or decline any health-related service. Routine Health-Related Services Provided at Your Child’s Campus:

In accordance with law, the District must notify parents of staff services related to school counseling and give parents the opportunity to opt out before those services are provided to their child.School counseling services are more fully explained in board policy FFEA, and student mental health is covered in board policy FFEB. Information about threat assessments can be found in board policy FFB. All District employees:
Under state law, a child may consent to counseling for themselves as it relates to suicide prevention; chemical addiction or dependency; or sexual, physical or emotional abuse. Routine Counseling ServicesState law requires school counselors to provide the following services as part of a Comprehensive School Counseling Program:

Parental ConsentUnder state law, an employee or contractor of the District must obtain the written consent of a child’s parent before the employee or contractor may conduct a psychological or psychiatric examination or test, or psychological or psychiatric treatment (“mental health-related services”), unless the examination, test, or treatment is required under law.These terms are defined by law:

Should the District recommend additional mental health care or the completion of a form or questionnaire not attached, the form will be provided to the parent and consent obtained before the form or services are provided to the student. |
How to Complete/Update Consent Forms

Log in to Skyward Family Access If you do not know your login or password, click Forgot your Login/Password and a reset link will be sent to the email address we have for you on file. Family/Student Access should be selected for the Login Area. |
Once you are logged in you will click on Online Forms in the left hand column. Click Fill Out Form for each form displayed for each of your students |
All students will have a Consent for Counseling and Consent for Health Services Forms that will need to be completed in order for your student to receive services under Texas House Bill 12. All high school students will have a Human Sexuality Instruction Consent Form that will need to be complete. Please click on Fill Out Form to complete each form. Consent for any services can be changed during the school year. To change your consent, click on View. |
You will then click to mark as not completed and make changes. |

At bottom of form click Edit Step 1. |
Once changes are made click to complete step 1 and move to step 2. |
You will now click to submit the form. |
